Family caregivers, who are typically unpaid individuals providing care to elderly loved ones at home, play a crucial role in maintaining their loved ones’ well-being. However, this responsibility often comes with significant emotional, physical, and financial burdens.
A recent qualitative study conducted in Mekelle City, Ethiopia, from December 2022 to October 2023, aimed to understand these challenges more deeply. The study revealed several pressing issues that caregivers face in providing daily care, from communication difficulties to financial strains, medication management, and transportation hurdles.
One of the major challenges uncovered in the study was the difficulty caregivers experience in communicating with senior relatives. As people age, behavioral changes such as agitation, frustration, and cognitive decline can complicate interactions. Caregivers in the study described how these factors often led to verbal outbursts, forgetfulness, and confusion, making it challenging to meet the fundamental needs of their elderly family members. The emotional weight of these interactions can be overwhelming, as caregivers struggle to understand and communicate with their loved ones.
To cope with these challenges, caregivers adopted various strategies, such as using simple language, practicing patience, and even relying on non-verbal cues like gestures. One caregiver in the study used walkie-talkies and a bell system to overcome physical distance within their home, ensuring quick communication when necessary. These adaptive measures highlight the ingenuity and resilience of family caregivers, but they also point to the emotional strain that comes with constant caregiving responsibilities.
Caregiving often imposes a heavy financial burden. In the Mekelle study, caregivers mentioned the costs associated with medical supplies, medications, and home modifications as a significant source of stress. Some were forced to reduce working hours, leading to a loss of income, which increased the strain. The ongoing conflict in the Tigray region further worsened the situation, cutting off retirement benefits and limiting access to government aid.
Many caregivers had to rely on savings, loans from relatives, or assistance from community organizations to make ends meet. While these efforts provided temporary relief, the financial challenges remained a constant struggle, emphasizing the need for systemic financial support for caregivers.
Managing medications for seniors posed another significant challenge. Many older adults require multiple medications, each with its own dosage schedule and potential side effects. Caregivers in the study described the difficulties in remembering and administering medications correctly, especially when their elderly family members struggled with swallowing pills or sticking to strict schedules.
To address these issues, caregivers turned to various strategies such as creating medication charts, setting reminders, and seeking advice from healthcare professionals. Some participants also utilized pill organizers and alternative medication formats, such as liquids, to ensure their relatives could take their medications safely and comfortably. While these solutions helped, they also added another layer of responsibility to the caregivers’ already overwhelming workload.
Transportation issues further compounded the difficulties faced by family caregivers in Mekelle. Accessing healthcare services was a significant challenge, particularly during emergencies, due to the lack of reliable transportation options in the area. The ongoing conflict had worsened the situation, reducing the availability of ambulance services and making it harder for caregivers to transport their loved ones to medical appointments. Caregivers described the frustration of waiting for taxis to fill up before they could take their elderly loved ones to healthcare facilities, adding unnecessary stress to an already challenging situation.
The study also shed light on the critical support needs of family caregivers. Many caregivers expressed a desire for financial assistance to alleviate the burden of caregiving costs. Others emphasized the importance of receiving proper training and access to reliable information to improve their caregiving skills. Some participants suggested that healthcare professionals should provide more regular guidance and check-ups for both the caregivers and their elderly relatives.
Additionally, the caregivers highlighted the need for specialized institutions or homes for the elderly, which could provide professional care and support, relieving some of the burdens placed on family caregivers. Material assistance, such as medical supplies and mobility aids, was also mentioned as essential in ensuring the well-being of seniors at home.
